With considerable help from my friends Ronnie Barnes and Phil Derome I have compiled a long list of changes to the first printing. Many of these changes are not errors made by myself, the copyeditor or the printer; the list includes numerous minor alterations that improve the clarity of the text.

To know whether you have a first printing, look inside on the publication details page. If it says ‘reprinted with corrections [date] ’ you will not need to make these changes, and neither will you need to download the corrected spreadsheets below.
